    Deep learning-based fetoscopic mosaicking for field-of-view expansion

    PURPOSE: Fetoscopic laser photocoagulation is a minimally invasive surgical procedure used to treat twin-to-twin transfusion syndrome (TTTS), which involves localization and ablation of abnormal vascular connections on the placenta to regulate the blood flow in both fetuses. This procedure is particularly challenging due to the limited field of view, poor visibility, occasional bleeding, and poor image quality. Fetoscopic mosaicking can help in creating an image with the expanded field of view which could facilitate the clinicians during the TTTS procedure. METHODS: We propose a deep learning-based mosaicking framework for diverse fetoscopic videos captured from different settings such as simulation, phantoms, ex vivo, and in vivo environments. The proposed mosaicking framework extends an existing deep image homography model to handle video data by introducing the controlled data generation and consistent homography estimation modules. Training is performed on a small subset of fetoscopic images which are independent of the testing videos. RESULTS: We perform both quantitative and qualitative evaluations on 5 diverse fetoscopic videos (2400 frames) that captured different environments. To demonstrate the robustness of the proposed framework, a comparison is performed with the existing feature-based and deep image homography methods. CONCLUSION: The proposed mosaicking framework outperformed existing methods and generated meaningful mosaic, while reducing the accumulated drift, even in the presence of visual challenges such as specular highlights, reflection, texture paucity, and low video resolution

    A Combined EM and Visual Tracking Probabilistic Model for Robust Mosaicking: Application to Fetoscopy

    Twin-to-Twin Transfusion Syndrome (TTTS) is a progressive pregnancy complication in which inter-twin vascular connections in the shared placenta result in a blood flow imbalance between the twins. The most effective therapy is to sever these connections by laser photo-coagulation. However, the limited field of view of the fetoscope hinders their identification. A potential solution is to augment the surgeon’s view by creating a mosaic image of the placenta. State-of-the-art mosaicking methods use feature-based ap- proaches, which have three main limitations: (i) they are not robust against corrupt data e.g. blurred frames, (ii) tem- poral information is not used, (iii) the resulting mosaic suf- fers from drift. We introduce a probabilistic temporal model that incorporates electromagnetic and visual tracking data to achieve a robust mosaic with reduced drift. By assuming planarity of the imaged object, the nRT decomposition can be used to parametrize the state vector. Finally, we tackle the non-linear nature of the problem in a numerically stable manner by using the Square Root Unscented Kalman Filter. We show an improvement in performance in terms of robustness as well as a reduction of the drift in comparison to state-of-the-art methods in synthetic, phantom and ex vivo datasets

    Pruning strategies for efficient online globally consistent mosaicking in fetoscopy

    Twin-to-twin transfusion syndrome is a condition in which identical twins share a certain pattern of vascular connections in the placenta. This leads to an imbalance in the blood flow that, if not treated, may result in a fatal outcome for both twins. To treat this condition, a surgeon explores the placenta with a fetoscope to find and photocoagulate all intertwin vascular connections. However, the reduced field of view of the fetoscope complicates their localization and general overview. A much more effective exploration could be achieved with an online mosaic created at exploration time. Currently, accurate, globally consistent algorithms such as bundle adjustment cannot be used due to their offline nature, while online algorithms lack sufficient accuracy. We introduce two pruning strategies facilitating the use of bundle adjustment in a sequential fashion: (1) a technique that efficiently exploits the potential of using an electromagnetic tracking system to avoid unnecessary matching attempts between spatially inconsistent image pairs, and (2) an aggregated representation of images, which we refer to as superframes, that allows decreasing the computational complexity of a globally consistent approach. Quantitative and qualitative results on synthetic and phantom-based datasets demonstrate a better trade-off between efficiency and accuracy

    Technical note: Probabilistic visual and electromagnetic data fusion for robust drift-free sequential mosaicking: Application to fetoscopy

    The most eâ†μective treatment for Twin-To-Twin Transfusion Syndrome is laser photocoagulation of the shared vascular anastomoses in the placenta. Vascular connections are extremely challenging to locate due to their caliber and the reduced field of view of the fetoscope. Therefore, mosaicking techniques are beneficial to expand the scene, facilitate navigation and allow vessel photocoagulation decision-making. Local vision-based mosaicking algorithms inherently drift over time due to the use of pairwise transformations. We propose the use of an electromagnetic tracker (EMT) sensor mounted at the tip of the fetoscope to obtain camera pose measurements, which we incorporate into a probabilistic framework with frame-To-frame visual information to achieve globally consistent sequential mosaics. We parametrize the problem in terms of plane and camera poses constrained by EMT measurements to enforce global consistency while leveraging pairwise image relationships in a sequential fashion through the use of Local Bundle Adjustment. We show that our approach is drift-free and performs similarly to state-of-The-Art global alignment techniques like Bundle Adjustment albeit with much less computational burden. Additionally, we propose a version of Bundle Adjustment that uses EMT information. We demonstrate the robustness to EMT noise and loss of visual information and evaluate mosaics for synthetic, phantom-based and ex vivo datasets

    Deep Placental Vessel Segmentation for Fetoscopic Mosaicking

    During fetoscopic laser photocoagulation, a treatment for twin-to-twin transfusion syndrome (TTTS), the clinician first identifies abnormal placental vascular connections and laser ablates them to regulate blood flow in both fetuses. The procedure is challenging due to the mobility of the environment, poor visibility in amniotic fluid, occasional bleeding, and limitations in the fetoscopic field-of-view and image quality. Ideally, anastomotic placental vessels would be automatically identified, segmented and registered to create expanded vessel maps to guide laser ablation, however, such methods have yet to be clinically adopted. We propose a solution utilising the U-Net architecture for performing placental vessel segmentation in fetoscopic videos. The obtained vessel probability maps provide sufficient cues for mosaicking alignment by registering consecutive vessel maps using the direct intensity-based technique. Experiments on 6 different in vivo fetoscopic videos demonstrate that the vessel intensity-based registration outperformed image intensity-based registration approaches showing better robustness in qualitative and quantitative comparison. We additionally reduce drift accumulation to negligible even for sequences with up to 400 frames and we incorporate a scheme for quantifying drift error in the absence of the ground-truth. Our paper provides a benchmark for fetoscopy placental vessel segmentation and registration by contributing the first in vivo vessel segmentation and fetoscopic videos dataset.Comment: Accepted at MICCAI 202
